Seniors at a long-term care home like Lydia Healthcare face challenges including separation from loved ones, a new environment, mobility issues, pain, and a poor quality of life, all of which may lead to feelings of worry and despair. Through exercises and diet, this population will get a lot of benefits that will help them overcome their mental problems. The implementation of the activities takes 6 weeks period. The first stage of preparation, evaluation and development of the program covers the first week and the remaining 5 weeks are for implementation of the activities. This will help document a 5% reduction of mental health and psychological conditions in the 6 weeks of mental health management in Lydia’s long-term healthcare facility.

The seniors in the long-term care facility like Lydia Healthcare experience the challenge of being away from family, change of environment, mobility problems, pain, and low quality of life that trigger anxiety and depression. Long-term care facilities focus on treating the patient’s conditions, and limited interventions are directed toward their mental health. It is more detrimental to their mental health if the problem is not addressed, which might contribute to an extended hospitalization for mental illness.

Among the primary causes of disability, depression imposes heavy burdens on individuals, families, and communities. It’s anticipated that treating chronic depression will cost twice as much as hypertension and diabetes in terms of direct medical expenses(WHO, 2017). Residents’ health and quality of life are severely compromised when nursing home employees are inadequately trained in mental health interventions because of the high prevalence of depression among the elderly. Care for the elderly Mental health is essential, especially in the long-term facility (Matos Queirós et al., 2021). The residents of a nursing home may be at risk of acquiring depression or for experiencing a worsening of their existing mental health problems. According to studies, half or more of the elderly in long-term care facilities may have clinical depression or exhibit depressive symptoms (Hoben et al., 2019).

The statement of need does not describe what the problem is at your facility. What measurable trends are a problem that need to be addressed (% of patients reporting sleep problems, symptoms of depression,etc). This must be a numeric value in order to use as a baseline.

Activities are written broadly. In order to use in this plan, activities need to be specific (i.e. stretching sessions will be offered 3x/ week Mon-Wed-Fri at 11am). 

How will you educate and provide the dietary options described? 

In order to revise, you could use an outcome of at least 60% patients participate in at least two exercise sessions each week.  The evaluation would correspond.  A more defined set of activities for how the exercise will be provided and how patients will be educated about options and encouraged to participate daily.

Engagement strategies to secure support would be to:

This strategy will be cost effective to implement because it will eliminate high rates of suicide as well as increase census rate. I believe these strategies will be effective in getting everyone involved in the plan.

· Good communication channels between family, patient and the staffs.

· The team is composed of experts in the field of mental health, psychology, dietitian, and social work.

· A lack of awareness of the issue among key stakeholders, including mental ill seniors, their families, and the staff who support them (Vafaeinasab et al., 2022).

· A lack of resources to effectively engage all stakeholders in the issue.

· Poor implementation may result from uncertainty about the cost and services offered when engaging a personal trainer.

· If a recommendation calls for the use of a public or outdoor exercise facility, the availability and rules of that facility will determine how well the plan is carried out.

The facility’s expenses will include the cost of bringing on a personal trainer who comes in 2 times a week for 3 weeks. This will be an estimated cost of about $180.

A special assessment of seniors with mental illness who are also exhibiting depressive symptoms will be done by 10% of the workforce. Conduct and observe seniors as they work out for 30 minutes, three times a week, doing things like stretching and stair climbing. Assure the elderly have an adequate diet. At the end of each shift, all shifts will record and submit their report. After eight weeks, the entire report will be assessed to determine whether we succeeded in lowering senior resident with mental illnesses’ rates of depression to 5%. The number of individuals who have demonstrated improvement in engaging in the facility’s socially organized events will be used to measure this.
