ou are creating a generic assessment tool that could be applied to any youth program. However, you may want to consider using the work you did in Mod 2 on writing program outcomes to give you an actual program you can use here. If you use the Mod 2 assignment, you will have a mission statement (either the Sample Youth Program Mission Statement or another mission you used to complete the assignment) to work with and PLOs you can use for the evaluation instrument.

Your assessment tool will include two parts:

  1. Description of how you are assessing all parts of the program
  2. Evaluation instrument

The description (1-2 pages in length) explains how you plan to assess all facets of the youth program including the participants, staff, goals, effectiveness, and relationship to the community.  The evaluation instrument is a sample questionnaire or survey that lets you gather data on program effectiveness.

Part 1: Assessment Tool Description

Structure the 1-2 page description around the following items and use each of the bolded topics as headings for your final submission:

1) Key Stakeholders:  Think about the key participants in your program (the kids); parents/caregivers; staff; and any community members as appropriate (businesses, school districts, churches, rec centers).  Identify and describe who the key stakeholders are.

You should have a minimum of three stakeholders:  the kids, their parents/caregivers, and the staff.

Give a one or two-sentence explanation of each stakeholder you identify.

Example:

Youth participants – The kids in the program are from the two local middle schools that are designated as Title I. Ages of the participants range from 11 – 13.

2) Data Analysis: You need a plan for how you will analyze the data you collect from the surveys.  Include 1-2 paragraph that addresses how you will analyze the results. (You did a similar data analysis in Identifying a Need in Mod 1).

3) Sharing the Results: You need a plan for how you will share the results that you get from your questionnaires or surveys. Include a 1-2 paragraph explanation of how you plan to communicate the results of the program assessment to each of your stakeholders. (You did a similar explanation of sharing results in Mod 1).

Part 2: Evaluation Instrument:

You want to create a sample questionnaire or survey that gives your key stakeholders a chance to give you feedback. Use the sample survey template or create your own.

If you use my sample survey template, you are using a spreadsheet to generate the questions you would ask. This is the easiest way to collect the types of questions you need. I suggest this for practice.

The directions/specifics for Part 2: Evaluation Instrument (how many questions and what to include) are on the sample survey template. I did use 4 worksheets, so make sure you pay attention to the tabs at the bottom of the file.

NOTE:

In Mod 7, you will create the survey or questionnaire that you would send out to the stakeholders. For now, the focus is on writing the questions.

You have examples of different survey and questionnaire tools in your module content that you can use to see how a completed instrument might look in the final project.

NOTE:

The evaluation instrument for practice here and in Mod 7 will have 1) quantitative and 2) qualitative questions.

Quantitative questions follow a Likert scale. The questions will be like a survey with responses from strongly agree to strongly disagree.

The ample survey template for this assignment uses primarily quantitative questions (Likert scaled).

Qualitative questions are open-ended. These types of questions allow respondents (your stakeholders) to provide written feedback. You want a few of these in your survey instrument.

Example of open-ended:  Did we meet your expectations in the program? Please explain.

Adding “please explain” prompts the need for written feedback. If you do not ask for an explanation, you have made this a yes/no question (and it is no longer an open-ended question). Avoid creating situations with qualitative questions that allow for only yes/no.

Save the bulk of your question writing for Mod 7. Keep it simple here in Mod 5. Just show me you can write a few sample questions for both qualitative and quantitative questions.
