Project (Rotating nurses and clinical staff to sit with high risk fall patients on a Med-surg. unit)

Purpose:

The purpose of the evaluation plan is to assess the effectiveness of the research interventions put in place during the implementation stage. The evaluation plan also measures the extent, if any, of closing any research gaps. The evaluation plan may also be used to examine organizational effectiveness and its ability to sustain implemented changes.

Tasks:

By the due date assigned, submit a 3- to 4-page evaluation that measures the success of the implementation phase addressing the following items:

• Describe and assess the four (4) project outcomes included in your implementation plan to evaluate the success of the change process.

• Identify future challenges and opportunities inherent in this change process.
