BIG GEMS Report Instructions:

APRNs need to be aware of social determinants of health (SDOH) when assessing, diagnosing, counseling (teaching) and prescribing (medication, supplements, treatments, therapies, referrals, etc.) for their patients. This assignment demonstrates a clinical application of population health concepts.

Examine and report findings for a patient using the BIG GEMS framework. The patient can be one from your current nursing position, NP clinical rotation, a family member, friend or yourself. Some examples of health issues to be considered in the BIG GEMS framework include among others, hypertension, diabetes, coronary artery disease, obesity, rheumatoid arthritis, gastroesophageal reflux, etc.

The only condition that cannot be used is Asthma, as it is the example in the text. Use text example (pages 14-15) as an exemplar (excellent report).
